Workplace Breastfeeding Support Survey

The Healthy Environments, Active Living (HEAL) program of the Utah Department of Health and Human Services is conducting a statewide survey to learn more about breastfeeding policies and activities in the workplace.

Why is this survey important? The HEAL team will use your feedback to better understand the current landscape in Utah and to tailor our program’s future services and activities to better serve our community.

Who should participate? We encourage managers and employees to participate if they:

• Are currently supporting someone who is pregnant or breastfeeding.
• Have breastfed within the last six months.
• Plan to breastfeed in the next six months.       

Survey Details:

• Time: It takes approximately 10 minutes to complete.
• Privacy: This survey is anonymous. We do not ask for personal information unless you choose to provide your contact details to receive assistance with developing breastfeeding policies for your workplace.
